Registration Registering for Cruzbol is easy.  There are two ways that you can register.  You can register by mail or electronically.  If you received a brochure, simply fill it out and send it in.  If you would prefer, you can access our registration form by clicking the appropriate link below.  Please remember that in order for your child to participate in any of our sessions, we must have a medical release waiver form signed and recorded in our office.  Please make all checks payable to Carlos Cruz.  You can find all contact information by simply choosing Contact Us.
